We are an information, support and guidance service aiming to ensure you feel fully supported.

We have access and links with local services that may be right for you.

We help avoid unplanned hospital admissions by working with our colleagues and other health and social care professionals.

We work with our clinical team to provide;

  • Support with upcoming clinical appointments.
  • Access to health care in a simpler way.
  • Support after hospital discharge.
  • Advice and information on local services.
  •  Helping you learn how to manage your own health.
